In pituitary prolactinoma management, the dopamine agonist cabergoline serves as the primary frontline therapeutic agent through targeted activation of cell surface dopamine D2 receptors (DRD2). Despite initial therapeutic response in most patients, a distinct subset of adenomas develops resistance to cabergoline, presenting a substantial clinical challenge. Recent mechanistic investigations reveal that metabolic dysregulation, particularly elevated intracellular cholesterol levels, contributes significantly to acquired drug resistance cascades. Excess free cholesterol within the endoplasmic reticulum membrane triggers cellular stress networks, specifically activating the Integrated Stress Response (ISR) and driving cytoplasmic phase separation into dynamic non-membrane-bound organelles known as stress granules.
Stress granules act as biophysical translation arrest hubs that selectively sequester cytosolic mRNAs, translation factors, and key signaling proteins during acute cellular strain. Under hypercholesterolemic conditions in prolactinoma cellular models, accelerated stress granule nucleation entraps newly synthesized and recycling DRD2 proteins within macromolecular aggregates. This physical sequestration interrupts normal vesicular trafficking pathways, preventing DRD2 transport through the trans-Golgi network and endosomal recycling circuits to the plasma membrane surface. Consequently, cell surface DRD2 expression drops markedly under cholesterol stress. With diminished surface receptor availability, cabergoline engagement falls below the threshold necessary to activate downstream heterotrimeric Gi-protein subunit signaling. As a direct result, adenylyl cyclase remains uninhibited, maintaining elevated intracellular cyclic AMP (cAMP) levels and promoting sustained cell survival and prolactin secretion.
Pharmacological intervention using an ISRIB eIF inhibitor offers a direct and highly selective mechanism to dismantle this resistance axis. ISRIB functions as a symmetric small-molecule activator of eukaryotic initiation factor 2B (eIF2B), a decameric guanine nucleotide exchange factor (GEF) responsible for converting eIF2-GDP to eIF2-GTP. Under metabolic stress, phosphorylated eIF2α acts as a competitive inhibitor of eIF2B, reducing GEF activity, suppressing global translation initiation, and promoting liquid-liquid phase separation of stress granule core proteins. ISRIB bridges the regulatory dimer interface of eIF2B, stabilizing its active assembly state and restoring GEF function even in the presence of high eIF2α phosphorylation levels. By neutralizing the downstream metabolic impact of eIF2α phosphorylation, ISRIB effectively blocks stress granule assembly.
In vitro characterization in cholesterol-challenged MMQ prolactinoma cellular models demonstrates that treatment with an ISRIB eIF inhibitor at a 1 μM concentration abolishes intracellular stress granule accumulation. Quantitative immunofluorescence co-localization studies reveal the rapid dispersion of canonical stress granule marker proteins, such as G3BP1 and TIA-1, accompanied by the release of sequestered DRD2 protein complexes. Subsequent subcellular fractionation and cell surface biotinylation assays validate that ISRIB treatment restores DRD2 localization to the plasma membrane. Notably, surface receptor density returns to baseline levels equivalent to non-stressed control cells without altering total DRD2 mRNA or protein expression levels.
Restoration of cell surface DRD2 abundance re-sensitizes cabergoline-resistant prolactinoma cells to dopamine agonist therapy. Functional signal transduction assays indicate that co-treatment with ISRIB (1 μM) and cabergoline reinstates canonical Gi-protein coupling, leading to robust inhibition of adenylyl cyclase activity and a marked reduction in intracellular cAMP accumulation. Furthermore, proliferation assays confirm that ISRIB co-treatment restores cabergoline-induced cell cycle arrest and apoptosis in cholesterol-exposed MMQ cells. These observations demonstrate that metabolic cabergoline resistance in prolactinomas is driven by reversible physical receptor sequestration rather than permanent receptor downregulation or gene deletion. Targeting stress granule dynamics with ISRIB represents a compelling pharmacological strategy to reinstate target receptor availability and overcome dopamine agonist resistance in refractory pituitary tumors.
Reversal of Cabergoline Resistance via Stress Granule Disassembly

ISRIB functions as an allosteric activator of eIF2B that counteracts the Integrated Stress Response (ISR). In prolactinomas, excess cholesterol induces eIF2α phosphorylation and stress granule assembly, which sequesters the dopamine D2 receptor (DRD2) intracellularly. This sequestration prevents DRD2 plasma membrane localization and induces cabergoline resistance. ISRIB (1 μM) restores eIF2B activity, disassembles stress granules, and restores DRD2 to the cell surface, reversing cabergoline resistance in MMQ cells.
Cholesterol-Induced Stress Granule Assembly Entraps DRD2
High intracellular cholesterol levels induce cellular stress in MMQ prolactinoma models. Phosphorylation of eIF2α triggers liquid-liquid phase separation. This process leads to rapid stress granule nucleation. Immunofluorescence staining reveals co-localization of DRD2 within G3BP1-positive granules. Sequestration prevents DRD2 trafficking to the plasma membrane surface. Consequently, cell surface DRD2 abundance declines significantly under cholesterol stress. Reduced surface receptor density directly correlates with impaired cabergoline binding affinity.
ISRIB Restores DRD2 Membrane Density at 1 μM Concentrations
Application of the ISRIB eIF inhibitor at 1 μM reverses cholesterol-driven stress granule accumulation. ISRIB binds directly to eIF2B to maintain its guanine nucleotide exchange activity. This binding overcomes eIF2α phosphorylation-mediated translational suppression. Stress granule disassembly occurs within hours of treatment. Western blot analysis of isolated membrane fractions confirms the recovery of DRD2 cell surface expression. The ISRIB eIF inhibitor re-establishes DRD2 localization without altering total cellular DRD2 gene transcription.
Synergistic Reversal of Agonist Resistance in MMQ Prolactinoma Cells
Restoration of surface DRD2 reverses cabergoline resistance in vitro. Co-administration of cabergoline with ISRIB suppresses adenylyl cyclase activity in cholesterol-treated MMQ cells. Intracellular cyclic AMP levels decrease significantly relative to cabergoline monotherapy. Furthermore, cell viability assays demonstrate restored antiproliferative responses. These empirical results confirm that inhibiting stress granule assembly restores functional dopaminergic signaling. Thus, targeting the integrated stress response offers a clear strategy to overcome metabolic drug resistance.
